PLEASE complete each form as much as possible BEFORE PRINTING by pressing tab key or clicking mouse to change fields in the form. Sponsors: There are two ways in which to register for a Brazos Valley Walk to Emmaus.

You can register your pilgrim directly online at

the BVEC online database

or you can download and complete the forms located below.

If you use the forms below, the Walk to Emmaus or Chrysalis require that four forms must be completed and submitted to the Registrar. For the Walk to Emmaus the Pilgrim must complete the Pilgrim Application, Medical Release for Cross Roads Retreat, and the Release of Liability and the Sponsor must complete the Walk to Emmaus Sponsor Form. For a Chrysalis the attendee must complete the the Chrysalis Flight Application, Medical Release for Cross Roads Retreat, and the Release of Liability and the Sponsor must complete the Chrysalis Flight Sponsor Form.

For a Walk to Emmaus Package please fill out these four forms:

Medical Release Form for Cross Roads Retreat (pilgrims and team members)
Release of Liability Form (pilgrims and team members)

For a Chrysalis Flight Package please fill out these four forms:

Chrysalis Flight Sponsor Form
Medical Release Form for Cross Roads Retreat (pilgrims and team members)
Release of Liability Form (pilgrims and team members)

In order to work a Walk to Emmaus or Chrysalis as either inside team member or outside team member please complete the appropriate Team Service Application as well as the Medical Release for Cross Roads Retreat, and the Release of Liability.

Other Forms:

These forms DO NOT REQUIRE DOWNLOADING in order to be filled out. The forms can be printed after being filled-in OR printed as BLANK FORMS and filled-in by hand. Computer filled forms are preferred by our registrar(s) for legibility reasons BUT NOT REQUIRED. Once the forms have been filled in by computer or hand, they should be signed, dated and processed according to their instructions. Please keep a copy for your records. If you have the latest version of the full Acrobat software package, then you have the option of saving the form WITH your filled-in data.
